Care Management for Returning Combat Veterans

Case managers work closely with an interdisciplinary team to coordinate Veterans’ care as they transition from the DoD to the VA system, which translates to enhanced continuity and quality of care. Also, case managers serve as a lead point of contact within the VA to facilitate a Veteran’s integration into the VA Health Care System and civilian life.   A Case Manager’s aim is to partner with individual OEF/OIF/OND Combat Veterans to support a healthy return home. Care management services include: Helping Veterans navigate and fully utilize the VA health care system. Assisting Veterans with understanding and accessing their VHA/VBA benefits. Using a collaborative relationship with a Veteran to foster personal growth and enhance their personal effectiveness These services are available to assist Veterans with accessing healthcare services and obtaining information on many issues, which include: Employment Finances Transportation Strengthening ties to the community Relationship Concerns Problems with sleep.   If you would like to speak with an OEF/OIF/OND Case Manager to discuss the Transition Program’s services and how they may help you, please contact us.  Meet Your Team
