Patient Safety Concerns
You have the right to present concerns pertaining to patient care and safety at all Health Care System locations including Outpatient Clinics. These concerns should be directed to the Manager of the Service or Department that is providing such care. If you are unsure of whom to contact, any employee can direct you to the appropriate supervisor. Should it appear that your concern is irresolvable at this level, you may contact the Patient Safety Line at (520) 792-1450 Extension 6500 or the Patient Advocate located in Building 1 A at Extension 4933.
Any individual with a concern about patient/resident care and safety in the hospital should contact the hospital management. If your concern cannot be resolved through the facility, you may contact VA Healthcare-VISN 18 in writing at: Network Office VISN 18 (10N18), 6950 E. Williams Field Rd., Mesa, AZ 85212.
If at any time you feel that hospital management has not addressed or resolved the concern, you may contact the Joint Commission by calling (800) 994-6610, on line at http://www.jointcommission.org or at the following address:
