Mental Health Intensive Case Management (MHICM)

The MHICM program has the overall mission to assist Veterans with Serious Mental Illness to reach their personal recovery goals and optimize their health, quality of life, and community functioning.  This is accomplished with a team of Social Workers and RNs who case manage and provide interventions  such as  coordinating care , visiting in the community, providing counseling and education to Veterans and their significant others, accompanying Veterans to appointments,  assisting with medication and money management, connecting to community resources, providing  group and individual therapy, assisting with transportation and housing issues, providing crisis response services, and assisting  Veterans to move toward greater independence.
